Federal EASI Form changes coming in September

Magellan Federal* is making changes to the EASI Form for billing Federal EAP services.

Key change: the Federal EASI form will no longer include a member name field on printable forms.

Instead, members’ cases will be identified only by their MAT number, assigned at the time of referral/case registration. Federal EAP MAT numbers begin with “EAP.”

We will share more information about changes to the Federal version of the EASI Form soon.
